    THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND
    DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT
    CAREFULLY. WE ARE REQUIRED BY LAW TO PROTECT HEALTH INFORMATION ABOUT YOU.

    We are required by law to protect the privacy of health information about you and that identifies you. This information may be information about services we provide you or payment for those services. It may also be information about your past, present, or future health status.


    We are also required by law to provide you with this Notice of Privacy Practices to explain our legal duties and privacy practices with respect to health information. We are legally required to follow the terms of this notice. In other words, we are only allowed to use and disclose health information in the manner that we have described in this notice.


    We may change the terms of the Notice in the future. We reserve the right to make changes and to make the new Notice effective for all health information that we maintain. If we make changes to the Notice, we will:
    • Notify you of the new Notice via the email address you have provided to us
    • Have copies of the new Notice available upon request

    • Explain your rights with respect to health information about you
    • Describe how and where you may file a privacy-related complaint

    WE MAY USE AND DISCLOSE HEALTH INFORMATION ABOUT YOU IN SEVERAL CIRCUMSTANCES.


    We use and disclose health information about clients every day. This section of our Notice explains in some detail how we may use and disclose health information about you in order to provide services, obtain payment for those services, and operate our business efficiently. This section then briefly mentions several other circumstances in which we may use or disclose health information about you. For more information, contact our Privacy Officer at 808.639.9221.

    We may use and disclose your health information only for each of the following purposes:
    • providing services
    • payment
    • business operations

    Providing services means coordinating information between two or more practitioners within the company who may at different times be working with you, such as sharing session notes from previous sessions in order to facilitate the best care for you.

    Payment means obtaining reimbursement for services, sending invoices when needed, and processing payments. An example would be processing your credit card you have authorized us to use for payment, or sending an invoice through PayPal or another online payment service.

    Business operations include the business aspects of running our practice, such as
    internal review of services for quality assessment and improvement activities. An
    example would be an assessment of services provided to assess and look for ways to provide better customer service.

    We may also create and distribute de-identified health information by removing all reference to individually identifiable information.

    We may contact you to provide appointment reminders or information about other services that may benefit you or be of interest to you.

    We may contact you to give you access to the session notes for each session provided to you, or to your minor children.

    You have the right to request that any of your health information held by us be provided to you at any time. This request can be made verbally or via email or letter and will be responded to within no less than one week.
